A fresh applicant, someone renewing, a parent applying for a child, and someone whose passport was lost all need different proofs and, in some cases, specific annexures. Find your situation

Fresh passport (first-time adult)

  • Proof of present address — Aadhaar, utility bill, rent agreement, bank passbook or similar.
  • Proof of date of birth — birth certificate, Aadhaar, PAN, or a school leaving certificate.
  • Aadhaar and PAN for identity.
  • Any annexure required for your particular circumstances.

Passport renewal / re-issue

  • Your existing (old) passport — original plus self-attested copies of the first and last pages.
  • Proof of present address if it has changed since the last passport.
  • Any document supporting a change you want on re-issue (name, address, appearance of new pages).

Minor's passport (child under 18)

  • Child's birth certificate.
  • Both parents' passports or identity/address proof.
  • The applicable parental-consent annexure — the exact one depends on the family situation.
  • Recent photographs as specified for minors.

Address change

  • Existing passport and proof of the new present address.

Name change (after marriage, gazette, etc.)

  • Existing passport plus the document evidencing the new name — marriage certificate, published gazette, or the relevant annexure.
  • A published gazette entry is often the cleanest proof for a name change.

Lost or damaged passport

  • Police report / FIR for a lost passport.
  • The damaged passport itself, where available.
  • The specific affidavit/annexure these cases require, plus your address and DOB proofs.

Passport checklist — common questions

Is there one document list for all passports?
No — that is the common mistake. Passport requirements vary by category. A fresh application, a renewal, a minor's passport and a lost-passport case each need a different set, which is why this checklist is sorted by scenario.
Which annexure do I need?
It depends on your exact situation — parental consent for a minor, or a specific declaration for a lost passport, for example. The official annexures are on the Passport Seva site; tell us your case and we will point you to the right one and help you complete it.
Do you charge for the passport itself?
No. Our assistance fee is ₹499 for helping you prepare documents and book the appointment. The government passport fee is separate, set by Passport Seva, and depends on the booklet type and Normal vs Tatkal.
Can I use a gazette to change my name on my passport?
Yes, a published gazette is a widely accepted proof of a name change for a re-issue. We handle both — the gazette first, then the passport re-issue — as a single sequence if you want.
My address proof is in a family member's name — is that a problem?
Often it can be handled with an additional document or annexure, especially for present-address proof. Send us a photo and we will tell you whether it works or what to add.
