What a passport annexure actually is
A passport annexure is simply a standard form — a declaration, affidavit or certificate — that the passport office asks for in specific situations. The main application captures your core details; annexures fill the gaps that only apply to some people. A parent declaring consent for a child, someone explaining how a passport was lost, a couple confirming a marriage-related surname change, a government employee giving a no-objection: each has its own annexure.
Think of them as a menu of pre-written statements, each tuned to one scenario. You do not need all of them — you need the one or two that match your case. The difficulty for most people is not filling an annexure in; it is identifying which one applies, because the letters look interchangeable and the online guides disagree. Why the annexure letters keep changing
Treat the letter as a signpost, not gospel
The passport office periodically revises and re-letters its annexures as rules are simplified — over the years the number of required annexures has been reduced. This means a letter quoted confidently on a blog from a few years ago may no longer match the current form. Throughout this guide we give the historical letters for orientation and clearly mark them as such, but the rule is the same in every case: download the current specimen for your situation from the official portal and go by that.

Common annexure mistakes
Almost every annexure problem we see falls into one of these:
- Using the wrong scenario's form. The classic error — for example a both-parents consent form when only one parent is available. Match the annexure to the real situation, not the one that is easiest to complete.
- Relying on an outdated letter. A form downloaded from a stale third-party page that the portal has since revised.
- Details that do not match the application. Names, dates and particulars on the annexure must agree exactly with the main form and your documents — the same word-for-word discipline that prevents a bounced file.
- Missing notarisation or signatures. Some annexures are affidavits or joint declarations with specific signing and, where applicable, notarisation requirements. An unsigned or improperly executed annexure is incomplete.
- Forgetting the accompanying document. An affidavit annexure for a lost passport, for instance, usually travels with a police report; the annexure alone is not enough.
None of these are hard to avoid once you know the correct annexure and read it carefully — which is exactly why identifying the right form up front matters more than anything else.

Two commonly needed specimens are published directly as PDFs: Annexure D (minor consent) and Annexure J (marriage joint declaration).
