What every gazette application needs, whatever the reason
Reasons for changing a name differ — marriage, divorce, correction, personal choice — but the spine of the application file is the same. Reported practice for the Gujarat route builds on a notarised affidavit supported by identity proofs, and everything else is scenario-specific evidence layered on top.

- Identity proof, self-attested copy — Aadhaar is the usual choice; PAN, passport, voter ID or driving licence also work.
- Address proof, self-attested copy — Aadhaar, utility bill, passport or similar showing your current address as it will appear in the notification.
- Two recent passport-size photographs — plain background, of the kind accepted on government forms.

Changing a first name, adjusting a spelling, adopting or dropping a surname by choice. Usually nothing beyond the core set — but bring the documents that show the old name (school certificate, Aadhaar, PAN) so the "changed from" spelling is anchored to a real record. Minor's name change
A parent or legal guardian applies on the child's behalf and swears the affidavit. Where parents are separated, be prepared to show the applying parent's authority for the change.

A rule we ask every client to remember: no legitimate gazette process requires you to surrender your original Aadhaar, PAN or certificates to anyone — including us. When originals such as the affidavit pass through our office, they are logged, stored securely and returned or submitted exactly as agreed.
When you do NOT need this page at all
If your documents already match — one name, one spelling, everywhere — you have nothing to gazette. And if the mismatch is a single-character typo on one document only, a direct correction with that issuer may be simpler than a gazette. Photograph and ID rules worth knowing
- Photographs: two recent passport-size prints on a plain background. Old photos invite avoidable questions at scrutiny; get a fresh set for a few rupees.
- Self-attestation: sign across each photocopy and write "self-attested". It is your personal declaration that the copy is true — no notary is involved in this step.
- Consistency beats quantity: the press does not want every document you own; it wants proofs that agree with each other. Two clean, consistent proofs outperform six contradictory ones.
- The old name is anchored to your proofs: the notification records a change from the name your documents show — so the affidavit's old-name spelling must match your strongest existing record, not your memory of it.
- Central gazette differences: the Gazette of India route additionally prescribes a proforma signed with two witnesses and its own format rules, and applicants must be 18 or older — one of several reasons we confirm your route before listing documents.
